Provides
in home counseling and case management to frail elderly and their families to help older
people remain independent. The staff assists seniors to identify and connect with the
right combination of services. Helps older adults access the most appropriate and cost
effective services available from a wide array of programs.
Support
groups for seniors and caregivers are available also. Call Barbara Lopata or Paulla

| Care Givers Support Group |
An opportunity for those who are caring for disabled family members to
meet with other caregivers in supportive conversation which will include practical
information, tips on self-care, good humor, and sharing of caregivers experience.
When: Two Thurs. each month

| Metro Mobility Program/strong> |
| Metro Mobility
is the door-through-door public transportation system for persons with disabilities in the
Minneapolis-St Paul area and certain adjoining suburbs. Certified riders call their Metro Mobility transportation
provider to schedule their trips. Trips can be scheduled up to four days in advance and
can be for any purpose. Metro Mobility is a shared ride public transportation system.
Routes and schedules are structured to transport multiple passengers to multiple
destinations. All trips are treated equally, whether a rider is traveling to a doctor's
appointment, going shopping, or meeting friends for dinner.
